Job Summary:

As a Home Health LVN, you will be responsible for conducting home health visits, providing health assessments, and administering medical care to our patients. You will work independently to drive to patient's homes, conduct assessments, and provide medical care as needed.

Key Responsibilities:

  • Conduct home health visits and provide health assessments to patients
  • Administer medical care, including immunizations and medication
  • Obtain and document medical histories
  • Complete documentation of assessment and/or interventions of patient
  • Perform venipuncture and other diagnostic tests as required
  • Administer vaccines and medication to patients
  • Monitor patient's progress and changes in behavior
  • Recognize and treat medical conditions with appropriate techniques and equipment
  • Interact with patients and their families in a compassionate manner
  • Communicate effectively with clinicians, other health professionals, and staff

Requirements:

  • LVN License
  • Excellent verbal communication skills
  • BLS with American Heart Association
  • Valid CA Driver's License and acceptable driving record
  • Patient bedside experience preferred
  • Spanish speaking (preferred but not required)
  • Must be comfortable driving and making home health visits independently
