About the Role:

As a Home Health Physical Therapist Assistant, you'll work as part of a multidisciplinary team to provide personalized care to patients in their homes. Your responsibilities will include:

  • Providing Home-Based Physical Therapy - Help patients regain their strength, manage their pain, and reach their goals in the comfort of their own homes.
  • Working with a Home Health Team - Collaborate with RNs, LPNs, CNAs, HHAs, OTs, and SLPs to develop and implement patient care plans.
  • Observing and Documenting Progress - Monitor patient progress, document outcomes, and modify care plans as needed.
  • Assisting Patients with Exercises and Therapy - Help patients manage pain and increase mobility through hands-on therapy and exercises.
  • Educating Patients and Families - Teach patients and their families about their care plans, therapy goals, and self-care techniques.
Requirements:
  • Active PTA License in Maine - Ensure you meet the necessary licensure requirements for the state of Maine.
  • Minimum of 1 Year of PTA Experience - Bring your experience and skills to our team.
  • CPR Certification - Stay up-to-date with the latest CPR certification requirements.
  • Up-to-Date Vaccinations - Ensure you meet the necessary vaccination requirements for the state of Maine.
  • Knowledge of State and Federal Home Health Regulations - Stay informed about the latest regulations and guidelines in home health care.
