Hospice LVN/LPN Care Specialist

Guaranteed Health is seeking a compassionate and skilled Hospice LVN/LPN to join our team. As a Hospice LVN/LPN, you will provide exceptional care and comfort to patients and their families during their end-of-life journey.
Responsibilities- Develop and implement individualized plans of care for patients, ensuring their needs are met and their comfort is prioritized.
- Provide accurate and timely documentation of patient services, reflecting the plan of care and ensuring seamless communication with the interdisciplinary team.
- Assess and educate patients and their families/caregivers on relevant topics, including diagnosis, self-care, and symptom management.
- Participate in the coordination of hospice services, identifying patient needs and reporting to the interdisciplinary group.
- Manage pain and symptoms effectively, evaluating patient responses to treatments and medications.
- Contribute to Guaranteed Health's quality assessment and performance improvement initiatives.
- Current licensure as an LPN/LVN in the state of application.
- Minimum 3 years of nursing experience, with a focus on community health, hospice, or medical/surgical care.
- Excellent communication, organization, and time management skills.
- Ability to work independently and as part of a diverse team.
- Desire to continuously learn and grow, with a commitment to ongoing education and professional development.
- 3 years of hospice experience (LVN, CNA, etc.).
- Experience conducting virtual visits.
