About the Role:
We are seeking a skilled RN to join our team at the Community Hospice House in Merrimack, NH. As a Hospice Nurse, you will have the opportunity to work with patients and their families to provide dignified and peaceful end-of-life care.
Responsibilities:
- Accessing, collecting data, and evaluating the patient's physical, spiritual, and emotional needs.
- Provides preventative and therapeutic treatment under medical direction.
- Creates a nursing plan of care with input from the patient, family, and provider to include measurable goals and behavioral objectives.
Qualifications:
- Graduate of an accredited school of nursing.
- Associate's degree in nursing.
- Strong skills in symptom management.
- Experience managing oncology patients and those with co-morbidities.

Two new opportunities: one is flex 3p-11p & 7a-3p for 32 hrs. a week and one is 3-11 for 32 to 40 hrs. a week.
