- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner, using electronic medical records (EMRs) to track patient data and outcomes.

- Contribute to the growth and development of the organization by participating in quality improvement initiatives and providing feedback on processes and procedures.
- Graduate of a physical therapy program approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Current, active license to practice physical therapy in the state of Florida.
- Minimum of two years' experience as a physical therapist in a home health care setting.
- Current CPR certification.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
