We are seeking an experienced Advanced Practice Provider (APP) to join our team of compassionate healthcare professionals. As an APP, you will play a critical role in delivering primary, urgent, and acute medical care to our patients in the comfort of their own homes.

Our APPs work collaboratively with our physicians, social workers, paramedics, and care team coordinators to provide concierge-level care to our patients. We offer a positive, upbeat work environment where all medical personnel work together for the good of the patient.

Responsibilities
  • Deliver primary, urgent, and acute medical care to patients in their homes
  • Collaborate with our interdisciplinary team to provide comprehensive care to our patients
  • Participate in regularly scheduled team meetings to discuss patient care and share best practices
  • Develop and implement individualized care plans for patients
  • Provide education and support to patients and their families
  • Document patient care and maintain accurate and current patient records
Requirements
  • Master's degree in Nursing
  • Valid nurse practitioner or physician assistant license in the state of operations
  • 5+ years of progressive care experience
  • BLS and ACLS certification
  • Ability to lift up to 20lbs and stand for extended periods
