At SpringHealth, we take a holistic approach to treating the mind and body together. Our services are designed to support individuals with autism spectrum disorder (ASD), intellectual and developmental disabilities (IDD), or mental illness to achieve their best overall health and well-being.
Responsibilities- Conduct ongoing assessments to meet the needs of patients served
- Communicate with supervisors, direct care staff members, family members, guardians, and collateral individuals regarding an individual's progress
- Provide a tentative schedule to supervisors of appointments, home visits, and observations for to monitor work advancement
- Coordinate home visits to meet the requirements of in-home observations, chart reviews, staff interviews, and data analysis
- Ensure implementation of the program plan is effective and properly carried out
- Participate in Interdisciplinary Team Meetings and other pertinent meetings as required
- Complete monthly and quarterly progress notes
- Organize trainings to certify staff and other team members are competent on the implementation of the program plan and/or documentation sheets
- Provide follow-up interviews, home visits, and observations for continued support of an individual's needs
- Maintain records to guarantee information is readily available to interested parties
- Act as liaison with the public and mental health community to provide support with development goals
- Assist in creating procedures for training materials to improve programs for individuals
- Experience with behavioral intervention, and/or a strong educational emphasis on the principles of learning and developmental psychology
- Two or more years of experience working with individuals with developmental disabilities or behavioral challenges
SpringHealth Behavioral Health and Integrated Care offers a holistic approach and integrates care for people with cognitive, developmental or intellectual disabilities who often need additional resources. Our team of behavior analysts, therapists, social workers, counselors, and psychologists work together to deliver high-quality behavioral services for clients to live more positive, active, and social lives.
Additional Job Information- MUST HAVE LPA WITH INDEPENDENT LICENSURE
- MASTERS DEGREE REQUIRED
- Any amount of experience with IDD clients in ICF and Waiver programs is a plus, as this is 95% of our client population. Those with counseling experience will have an opportunity to provide therapy services.
