About the Role

Fallon Health Weinberg is seeking a skilled Primary Nurse to join our team. As a Primary Nurse, you will be responsible for providing high-quality, patient-centered care to our PACE participants.

Key Responsibilities
  • Advocate for the best care for each participant, utilizing the nursing process and adhering to standards of nursing practice.
  • Foster relationships with caregivers and participants, promoting a collaborative approach to care.
  • Lead the healthcare team, effectively communicating with colleagues and ensuring tasks are completed.
  • Develop and implement individualized plans of care, taking into account the unique needs and circumstances of each participant.
  • Conduct regular assessments, communicate with the care team, and coordinate care to ensure optimal outcomes.
  • Provide culturally sensitive care, addressing the diverse needs of participants.
  • Monitor and evaluate therapeutic interventions, making adjustments as needed.
  • Delegate and accept care responsibilities, ensuring seamless transitions and continuity of care.
  • Identify and respond to emergency situations, initiating appropriate nursing orders and interventions.
  • Meet the needs of participants in a timely and efficient manner.
  • Participate in the interdisciplinary team process, collaborating with colleagues to meet the needs of participants.
  • Consistently document all aspects of participant care, including significant changes in health status and health care teaching.
Requirements
  • Graduate of an accredited school of nursing.
  • Current license as an RN in the state of New York and CPR Certification.
  • At least two years of recent experience in the direct care of adults or chronically disabled persons, with at least one year caring for a frail or elderly population.
  • Ability to use an electronic medical record.
