As a Licensed Practical Nurse (LPN) on our Long-Term Care team at LifeBridge Health, you will play a vital role in providing high-quality patient care to our residents. You will work under the supervision of a Registered Nurse/Director of Nursing to develop and implement individualized nursing care plans that meet the unique needs of each resident.
Responsibilities:
- Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities.
- Conduct thorough assessments of residents' physical and emotional needs, and develop and implement individualized care plans to address these needs.
- Collaborate with the interdisciplinary team to ensure that residents receive comprehensive care that meets their physical, emotional, and social needs.
- Assume leadership roles as needed to ensure the smooth operation of the unit and provide guidance and support to colleagues.
- Participate in quality improvement initiatives to enhance the quality of care provided to residents.
Requirements:
- Graduate of an accredited school of nursing with a current LPN license.
- Basic computer skills and proficiency in electronic medical records.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
