Home Health Physical Therapist

We are seeking a skilled Physical Therapist to join our team at Ochsner Home Health of New Orleans, a part of LHC Group. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient well-being.
Responsibilities- Evaluate patient functional status and physical therapy needs
- Assist physician in developing therapy plan of care
- Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
- Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals
- Current Physical Therapy licensure in state of practice
- Current CPR certification
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
