GENERAL OVERVIEW:

The Integrated Case Manager-Resident plays a crucial role within the patient-centered care team, engaging in a collaborative approach to assess, plan, facilitate, coordinate care, evaluate, and advocate for a variety of options and services. This position is dedicated to addressing the comprehensive healthcare needs of individuals and families through effective communication and resource utilization, ultimately enhancing patient safety, care quality, and cost-efficient outcomes.

This role specifically focuses on patients who have undergone significant health events or diagnoses necessitating intricate management strategies and extensive resource utilization to optimize health outcomes throughout the care continuum. Services are provided in both outpatient and inpatient environments.

EDUCATIONAL BACKGROUND AND EXPERIENCE REQUIRED:

  • Master's degree in Social Work from an accredited institution is mandatory.
  • Proficient in assessing and identifying barriers while collaborating with care team members to address patients' social and clinical requirements.
  • Excellent verbal communication and written documentation capabilities.
  • Exceptional customer service and interpersonal skills, enabling effective interaction with all members of the patient's care team.
  • Strong analytical, problem-solving, and decision-making abilities.
  • A deep sense of compassion and the capacity to advocate effectively for patients and their families.

CERTIFICATIONS AND LICENSES REQUIRED:

  • Possession of a Limited Licensed Social Worker (LLMSW) designation with a valid, unrestricted license in the State of Michigan.
  • Must achieve full licensure (Licensed Master Social Worker-LMSW) within one year of completing 4 supervised hours.
