Palliative Care Nurse

Job Summary
We are seeking a skilled Registered Nurse to join our team as an Advanced Illness Management RN. In this role, you will work with patients, their families, and other healthcare professionals to provide and coordinate high-quality end-of-life care.
Responsibilities
- Conduct in-person and telephonic assessments to palliative care patients in collaboration with the interdisciplinary team.
- Provide education to patients and families on disease process, self-care techniques, end-of-life care, medications, nutrition, and dietary needs.
- Communicate any findings that identify appropriate preventive and rehabilitative skilled nursing or therapy needs to the PCP or AIM Palliative NP.
- Coordinate care with palliative team members and communicate change in condition or signs and symptoms of decline to Palliative NP and/or PCP.
- Support hospice referral as appropriate and document timely clinical records on all patients.
Requirements
To be successful in this role, you will need:
- 2 years of prior skilled Registered Nurse experience.
- Demonstrated ability to provide quality patient care.
- Nursing experience in palliative care, oncology, hospice, or geriatric care preferred.
