Responsibilities- Conduct thorough patient assessments and develop individualized care plans to meet the unique needs of each patient.
- Manage a caseload of patients, ensuring timely and effective care coordination with other healthcare professionals.
- Collaborate with physicians, caregivers, and insurance companies to ensure seamless care delivery.
- Minimum one year of experience in home health and hospice care within the last year.
- Proficient knowledge and field experience with electronic medical records systems.
- Start of Care/Admissions experience required.
- Strong case management skills, with the ability to handle a high volume of patients.
- Associate's Degree of Nursing or Bachelor's Degree of Nursing.
- Pennsylvania Registered Nurse License or compact license.
- American Heart Association Basic Life Support certification.
