The Licensed Practical Nurse (LPN) plays a vital role in delivering skilled, empathetic nursing care to patients and their families under the direct oversight of a physician and Nurse Manager.
The LPN collaborates with healthcare professionals to maintain a quality-driven, patient-centered workflow within the practice, while also serving as a knowledgeable resource for both clinical and administrative support staff.
KEY RESPONSIBILITIES:
Provides essential physical care and emotional support to patients and families under the supervision of the Nurse Manager and Medical Director.
Engages in the planning, execution, and assessment of nursing care and treatment strategies; meticulously observes, reports, and documents relevant patient information in the electronic health record system, adhering to established protocols; identifies patient care requirements and effectively utilizes nursing resources.
Administers medications in accordance with established guidelines after successful orientation; performs nursing procedures as defined by the service protocols.
Participates in Quality Assurance initiatives; supports colleagues in upholding established care standards.
Identifies patient care needs and efficiently utilizes nursing resources.
Handles urgent clinical calls, which may involve scheduling patient appointments, including Telehealth sessions.
Communicates pathology, lab results, and care plans to patients.
Monitors all biopsy results through In-Basket messages, ensuring timely administration of necessary therapies as scheduled by the physician.
Obtains prior authorizations for medications as required.
Contributes to the development and implementation of innovative nursing practices; engages in daily activities that enhance the comfort and safety of patients, visitors, and staff.
Offers insights to the Nurse Manager regarding perceived unit needs. Participates in unit and service committees.
Ensures maintenance of the physical environment.
Implements and enforces annual fire and safety regulations for the practice, following established protocols, and ensuring compliance with relevant guidelines.
Addresses routine and non-routine issues, taking appropriate measures to resolve them.
The individual in this role must demonstrate a comprehensive understanding of growth and development principles across the patient lifespan.
Additionally, the candidate must possess the ability to assess patient data concerning age-specific needs and deliver care in accordance with departmental policies and procedures.
Considers feedback from patients, families, and visitors, relaying insights to the Nurse Manager for continuous improvement.SKILLS AND COMPETENCIES:
Maintains and enhances clinical knowledge and skills.
Engages in relevant continuing education opportunities.
Participates in Nurses Association meetings.
Demonstrates a commitment to delivering the highest quality patient care and exceptional service to all patients.
Exhibits initiative and the ability to exercise independent judgment, decision-making, and problem-solving skills.
A collaborative, team-oriented attitude is essential, alongside the capacity to work independently and interact effectively with physicians, managers, and staff throughout the organization.
Administration of injections is required.
LICENSES AND CERTIFICATIONS:
Current state licensure as a Licensed Practical Nurse is mandatory.
CPR certification is required.
BLS certification is necessary.
Phlebotomy experience is strongly preferred.
EDUCATION:
Graduation from an accredited LPN program is required.
EXPERIENCE:
A minimum of 2 years of LPN experience is preferred.
Education: High School.
