Social Services Coordinator

We are seeking a highly skilled and compassionate Social Worker to join our team at People's Self-Help Housing. As a key member of our Resident Services team, you will play a vital role in providing support and resources to our residents, helping them achieve long-term success and stability.
Key Responsibilities- Collaborate with property management staff and other personnel to provide comprehensive support services to residents.
- Solicit and respond to referrals from property management staff, residents, and community members to address challenges related to day-to-day functioning.
- Conduct client assessments, develop and oversee case management plans to ensure long-term success.
- Emphasize collaboration with other community agencies to provide a network of support services.
- Assist residents with referrals to community resources, benefit programs, and related services.
- Complete evaluations, plans, and service agreements with residents as needed.
- Document all face-to-face contacts, phone contacts, and activities related to service agreements.
- Maintain accurate service records in the SHP database.
- Accompany residents to appointments as required to ensure services are received.
- Provide crisis intervention, counseling, and case management services as needed.
- Refer residents to long-term therapy services in the community as needed.
- Assist with property activities related to Health and Community Services functions.
- Perform other duties as assigned by the Clinical Supervisor and/or Director of Resident Services.
- Ability to handle sensitive information in a confidential manner.
- Excellent communication and organizational skills.
- Computer skills, including Microsoft Word, Microsoft Outlook, and Excel.
- Ability to work with diverse cultures, ethnicities, and minorities.
- Ability to identify mental health conditions, diagnose, and provide treatment and/or appropriate referrals.
Preferred Qualifications
- Bilingual (English/Spanish) preferred.
Education and Certification
- Master's Degree in psychology, social work, or other relevant field.
License and Certification
- Valid CA Driver's License with access to a reliable vehicle.
Physical Requirements
- Sitting for long periods, walking, and stretching; occasional light lifting; travel via automobile.
- Travel may be required during regular business hours.
