Job SummaryThe Home Health Licensed Vocational Nurse will be responsible for monitoring assigned cases to ensure compliance with the requirements of third-party payors. This role requires strong communication and interpersonal skills, as well as the ability to work independently in a patient's home environment.
Key Responsibilities- Case Management: Participate in planning and implementing care in conjunction with the RN, by the Plan of Care. This may include wound care, medications, and upcoming vaccinations.
- Patient Assessment: Under the direction of the RN, assist in identifying the patient's physical, psycho-social, and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, and evaluations.
- Communication: Provide effective communication to patient/family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes.
- Education: High School Diploma – Required
- Certifications: Graduate of an accredited school of vocational nursing. – Required
- Experience: Must have Home Health Experience.
- Licensure: Current Texas State license as a Licensed Vocational Nurse. – Required
- Preferred Qualifications: 2+ years' experience as a Licensed Vocational Nurse in a clinical care setting; home health – Preferred
- Skills: Nursing skills as defined as generally accepted standards of practice. Must have good interpersonal skills.
- Additional Requirements: Reliable transportation. Valid and current Texas driver's license and valid and current auto insurance.
- Compensation: Starting at $35/hr. (based on experience) + benefits.
- Benefits Package: Paid Mileage, Full benefits – healthcare, dental, vision, 401k program.
