We are seeking a skilled Speech Language Pathologist to join our team at Interim HealthCare Home Health and Hospice. As a Speech Language Pathologist, you will play a vital role in providing high-quality care to our patients.
Key Responsibilities:- Conduct comprehensive assessments to identify patient needs and develop personalized plans of care.
- Provide speech and language therapy services to patients, including counseling and education to promote progress and independence.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and effective communication.
- Document patient progress and outcomes in accordance with professional standards and regulatory requirements.
- Graduate of an accredited Speech Language Program and active SLP license in Michigan.
- At least one year of experience as an SLP in the last five years.
- Excellent clinical judgment, strong interpersonal skills, and a compassionate approach to patient care.
