About the Role

In this Patient Care Coordinator position, you will have the opportunity to make a meaningful difference in the lives of our patients.

  • Evaluate patients/residents to collect data, observe conditions, and report changes in condition.
  • Contribute to nursing assessments and care planning.
  • Administer medications and perform treatments per physician orders.
  • Supervise and monitor patient care provided by unlicensed staff.
Requirements

To be successful in this role, you must be a graduate of an approved school for practical or vocational nursing and currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.

A minimum of 1-2 years of experience in a similar role is preferred, but not required. The ideal candidate will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
