We are seeking a skilled and compassionate Home Health Occupational Therapist to join our team. As an Occupational Therapist, you will work with patients to help them achieve their goals and improve their functional abilities.

Responsibilities:
  • Conduct initial evaluations and develop individualized plans of care for patients
  • Provide rehabilitative and skilled occupational therapy services to patients in their homes
  • Use various tests to determine patient needs and develop strategies to improve functionality and daily living skills
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Document patient progress and revise plans as needed
Requirements:
  • Graduate of an ACOTE-accredited occupational therapy education program
  • Current, unencumbered license to practice as an OT in this state
  • Minimum 1 year recent work experience as an OT
  • Valid driver's license and current automobile insurance
