About the Role

We are seeking a compassionate and skilled Clinical Social Worker to join our team at Georgia Hospice Care. As a Clinical Social Worker, you will play a vital role in providing emotional, spiritual, and practical support to patients and their families.

Key Responsibilities
  • Conduct thorough psychosocial evaluations to identify patient needs and develop personalized care plans
  • Provide ongoing counseling and support to patients and their families, addressing their emotional, spiritual, and practical needs
  • Collaborate with the interdisciplinary team to ensure comprehensive care and coordination of services
  • Develop and implement bereavement programs and services to support patients and their families
  • Participate in community education and outreach initiatives to promote hospice care and services
Requirements
  • Master's degree in Social Work from an accredited school of social work
  • Current state license as a social worker
  • At least 2 years of clinical work experience, preferably in healthcare or hospice
  • Reliable transportation and ability to travel to patient homes and facilities
