Job Summary

As a Registered Nurse with ComForCare Home Health Care - Lowell/Concord, you will be responsible for performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.

You will develop an individualized plan of care for each client that incorporates analysis of data and current scientific findings.

Providing interventions with a focus on achieving realistic client outcomes is a key aspect of this role.

You will report changes to update the plan of care to reflect progress towards goals and outcomes.

Administering medications as needed is also a part of this job.

Maintaining client records showing systematic assessments, planning interventions, and evaluations is crucial.

You will initiate referrals as needed to other health team members.

Requirements

This is a PER DIEM position.

Graduate of an accredited school of professional nursing is required.

Current license as a Registered Nurse (RN) is necessary.

Current CPR certification is also required.

Access to reliable transportation is a must.

Maintaining confidentiality in all aspects of the job is essential.

Knowledge of safety and infection control practices is necessary.
