Job Title: Senior Transition Specialist

Join Fidelis Care as a Senior Transition Specialist and play a critical role in transforming the health of our communities. As a key member of our team, you will be responsible for ensuring the timely and safe transition of members in the community from various levels of healthcare services.

Key Responsibilities:
  • Work with care coordination teams, providers, and other healthcare team members to develop effective transition plans for members in the community.
  • Assist with the transition of care for members in the community based on enrollment or transition of care for services identified.
  • Identify new member enrollees requiring transition services and work with care coordination and care management teams to address their needs.
  • Ensure existing authorizations are honored during the transition process and work with care management teams and providers to address any issues.
  • Act as a resource for members and their families/caregivers to educate them on services, requirements, limitations, and exclusions of services as a result of transition planning.
  • Review transition metrics, including new member assessments and volume of members transitioning into or out of care, to identify trends and process improvements.
  • Partner with leadership to identify process improvements for transition of care for members and provide recommendations.
  • Develop education materials and resources for members on requirements, limitations, or exclusions of services for transition of care.
  • Develop education and training programs for care coordination staff and providers to improve transition of care services for members.
Requirements:
  • Bachelor's degree and 4-6 years of related experience or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope, and skill reflective of the level of this position.
