Key Responsibilities
  • Patient Care and Assessment
    • Conduct comprehensive assessments to identify patient needs and develop effective treatment plans.
    • Provide crisis counseling and psychosocial support to patients and their families.
    • Collaborate with multidisciplinary teams to ensure seamless care coordination.
  • Referral and Linkage
    • Facilitate referrals to appropriate treatment programs and services.
    • Develop and maintain relationships with community partners to ensure access to resources and support.
  • Environmental Safety and Management
    • Ensure a safe and healthy environment for patients, staff, and visitors.
    • Identify and report potential safety hazards and take corrective action.
  • Information Management and Compliance
    • Collect and maintain accurate and confidential patient information.
    • Ensure compliance with regulatory requirements and organizational policies.
Requirements
  • Education and Licensure
    • Masters in Social Work (MSW) or related field.
    • Current licensure as a Licensed Counselor or Registered Nurse (RN).
  • Experience
    • Minimum two years' experience in a psychiatric setting.
  • Skills and Abilities
    • Excellent communication and patient assessment skills.
    • Ability to work in a fast-paced environment and manage multiple priorities.
    • Strong problem-solving and decision-making skills.
Working Conditions

This role requires the ability to work in a dynamic and sometimes challenging environment. You will be exposed to communicable diseases and may be required to wear personal protective equipment. You must be able to lift up to 25 pounds and stand for long periods.
