At Cynet Health, we are seeking a skilled Hospice Care Specialist to join our team. As a Hospice Care Specialist, you will provide high-quality care to patients in their homes, working closely with our clinical team to ensure the best possible outcomes.

Responsibilities:
  • Provide comprehensive home nursing care to patients, following physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Travel to patients' homes for visits and office meetings, requiring reliable transportation and a valid driver's license.
  • Manage a caseload of 5-10 patients per day, with an average of 5-8 patients per day.
  • Types of cases include start of care, wound care, follow-up visits, and more.
  • Participate in on-call rotations and weekend rotations, approximately 1 night a week and 1 weekend a month.
  • Travel up to 75 miles from the home clinic, with mileage reimbursed at the IRS standard rate.
  • Proficient in HomeCare HomeBase EMR system and OASIS, with experience using them within the last year.
  • Meet weekly productivity expectations of 25 units.
Requirements:
  • Minimum 1 full year of adult home health experience or 1 full year hospice experience.
  • Valid license in hand.
  • CPR and BLS certifications.
  • Proficiency in HomeCare HomeBase and OASIS within the last year.
