About the Role

As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care and support to clients with mental health and addiction issues. You will work collaboratively with a multidisciplinary team to develop and implement individualized treatment plans that address the unique needs of each client.

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues. You will also be comfortable working in a dynamic and rapidly changing environment, with a focus on delivering exceptional patient outcomes.

Key Responsibilities
  • Provide direct patient care and support to clients with mental health and addiction issues
  • Develop and implement individualized treatment plans in collaboration with the multidisciplinary team
  • Conduct thorough assessments and gather information to inform treatment planning
  • Communicate effectively with clients, families, and colleagues to ensure seamless care coordination
  • Participate in staff meetings and training sessions to enhance knowledge and skills
Requirements

To be successful in this role, you will need:

  • A current LPN license in the state of Ohio
  • Experience working in a mental health or addiction setting
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong problem-solving and critical thinking skills
