Position Overview

Penobscot Community Health Care is in search of an Advanced Practice Provider to enhance our established and expanding integrated family medicine practice. This role is pivotal in delivering a comprehensive array of services including primary care, urgent care, mental health support, and recovery services. Join a pioneering Federally Qualified Health Center (FQHC) committed to providing exceptional, all-encompassing care to our diverse patient population.

Key Responsibilities

  • Full-time commitment of 40 hours per week, with 34 hours dedicated to patient interactions.
  • Deliver a wide spectrum of primary outpatient care throughout various life stages.
  • Foster long-term relationships to ensure continuity of care.
  • Collaborate with a multidisciplinary team, including family medicine physicians, mental health professionals, and other specialists.
  • Implement integrated preventive care and educational initiatives.
  • Prior experience is preferred, but new graduates are encouraged to explore this opportunity.
