Job Summary

We're seeking a skilled LPN to join our Inpatient Hospice team. As a key member of our team, you'll provide high-quality patient care, work collaboratively with our interdisciplinary team, and contribute to creating a positive and supportive work environment.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements
  • Graduated from an approved school of professional nursing, Associates Degree preferred
  • One year experience as a Licensed Practical Nurse, preferred
  • Valid CPR certification
  • Ability to work in a fast-paced environment and prioritize tasks effectively
