About You:

  • Bachelor's degree in social work from an institution accredited by the Council on Social Work Education; Master's degree in social work preferred;
  • Licensed in the state they serve;
  • At least one year of experience in health care, preferably in hospice settings;
  • Strong verbal and written communication skills;
  • Able to pass DHS background study;
  • CPR certification;
  • Valid State Driver's License, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record (if applicable).

Responsibilities:

  • Assess psychosocial status of patients and families/caregivers related to terminal illness and environment;
  • Communicate findings to Registered Nurses and other interdisciplinary care team members;
  • Carry out social evaluations, including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plans intervention based on evaluation findings;
  • Counsel patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties;
  • Participate in development of individualized plan of care, involving patient and family, and attends regularly scheduled interdisciplinary group meetings;
  • Maintain compliance with all HIPAA regulations and requirements, treating all patient information as confidential and utilizing protected health information on a minimum necessary basis.
