**Requirements**

To be successful in this role, you will need:

  • A current license to practice as a Registered Professional Nurse in New York State.
  • Strong clinical skills and experience in patient care coordination.
  • Excellent communication and interpersonal skills.
  • Able to work effectively in a fast-paced environment.
