About the Role

We are seeking a highly skilled and compassionate Substance Abuse Counselor to join our team as a Hybrid Clinician. As a key member of our clinical team, you will play a vital role in providing comprehensive substance abuse counseling services to patients in a hybrid setting.

Key Responsibilities
  • Interface with the clinical team to make informed decisions regarding patient care.
  • Assess the counseling needs of patients and develop individualized treatment plans.
  • Conduct substance abuse assessments and develop treatment plans based on ASAM criteria.
  • Provide substance abuse counseling services, including individual and group therapy.
  • Develop and implement discharge/continuum of care plans to ensure smooth transitions to appropriate levels of care.
  • Document all counseling and education services, assessments, and follow-up in patient records.
  • Participate in multidisciplinary team meetings and contribute to the development of treatment plans.
Requirements
  • Bachelor's or Master's degree in psychology, counseling, or social work from an accredited institution.
  • Three years of experience in substance abuse, mental health, or a related field.
  • Licensure or certification in counseling, as required by the state.
  • Credentialed as a Licensed Clinical Social Worker (LCSW), Licensed Mental Health Counselor (LMHC), or other relevant credential.
