Job Overview

The Licensed Vocational Nurse, Clinical Support Partner plays a crucial role in implementing education processes for healthcare professionals, ensuring patient-centered care across all PACE and ACS locations.

Key Responsibilities
  • Conduct assessments, planning, implementation, and evaluation of competency assessments to achieve high-quality patient care.
  • Collaborate with the PACE Clinical Nurse Informatics Specialist and various PACE departments to develop standardized training materials.
  • Develop and implement cost-effective, efficient training protocols, workflows, processes, systems, and key initiatives for the PACE and ACS clinical nursing departments.
Requirements
  • Graduation from an accredited Licensed Vocational Nurse School.
  • Current unrestrictive LVN license issued by the State of California Vocational Nursing and Psychiatric Technicians.
  • Bachelor's degree in Nursing, preferred.
  • Current California Driver's License, required.
  • Current First Aid and BLS Certification, required.
  • Minimum of 1 year of experience working with frail elderly (Geriatric) population as required by CMS PACE regulations.
  • Minimum of 2 years of work experience in a PACE program is required.
  • Minimum of 2 years of hands-on training experience is required to develop training programs, procedures, and routines, preferably in healthcare organizations.
Preferred Qualifications
  • Experience with developing Learning Management System training modules.
  • Prior experience with an Electronic Health/Medical System, required.
Compensation and Benefits

$36.00 hourly, $65, $81,598.40 annually. AltaMed offers a competitive compensation package, including medical, dental, and vision insurance, 403(b) Retirement savings plans, and career advancement opportunities.
