About the Role:
Hearth Hospice is seeking a compassionate and skilled Spiritual Care Provider to join our team in the Chattanooga, TN area. As a member of our Interdisciplinary Team, you will provide spiritual counseling and emotional support to patients and their families, promoting a holistic approach to care.
Key Responsibilities:
- Assess the spiritual needs of patients and families, communicating findings to the RN Case Manager and interdisciplinary team.
- Provide direct spiritual care to patients and families of all faiths and backgrounds, addressing their unique needs and concerns.
- Maintain regular communication with the Program Director to review client needs and coordinate spiritual care services.
- Initiate communication with hospice staff and other facilities as needed to ensure optimal care and resource utilization.
- Monitor and maintain accurate documentation of clinical records, ensuring compliance with regulatory standards.
Requirements:
- Graduate of an accredited seminary or school of theology, or equivalent certification in pastoral ministry.
- Minimum two units of Clinical Pastoral Education, with four units preferred.
- At least two years of experience as a healthcare pastor or chaplain, with one year of hospice experience preferred.
- Ability to handle stress associated with ministering to patients and their loved ones.
- Proficiency in computer typing and various programs, including EMR.
- Current CPR certification, if required by state.
