Role Overview:

The primary objective of this role is to deliver diagnostic and therapeutic services aimed at assessing, treating, documenting, and facilitating care for residents experiencing difficulties with swallowing or communication due to medical conditions. This position also involves collaboration in interdisciplinary team meetings and assisting in the development of policies to prevent communication and swallowing decline.


• Assess residents' functional requirements upon referral from their attending physician or interdisciplinary team.


• Adhere to physician directives, facility protocols, and documentation standards while evaluating and treating residents.


• Collaborate with the Director of Nursing Services to formulate policies and procedures aimed at preventing communication and swallowing decline.


• Engage in discharge planning as necessary.


• Conduct diagnostic assessments and document evaluations of referred residents in compliance with facility standards.


• Deliver suitable treatment to assist residents in achieving or maintaining optimal communication and swallowing capabilities within their physical limitations.


• Recommend and/or provide effective treatment strategies, preventive measures, and assistive devices to avert functional decline.


• Educate residents or caregivers to support the maintenance or improvement of communication and swallowing abilities.


• Ensure timely and accurate reporting of resident evaluations as mandated by regulatory agencies and third-party payers.


• Follow infection control protocols diligently.


• Maintain the safety of residents under supervision.


• Complete assigned duties related to Minimum Data Set assessments.


• Document evaluations, treatment plans, care plans, and progress notes promptly.


• Participate in resident care and interdisciplinary conferences as scheduled.


• Contribute to the implementation of programs aimed at enhancing resident care related to Quality Indicators.


• Undertake Quality Assurance responsibilities as assigned.


• Comply with all facility policies and procedures.


• Adhere to safety protocols as outlined by the facility.


• Follow the guidelines established in the employee handbook.


• Respect the Residents' Rights policies of the facility.
