Geriatric Care Coordination Therapist

We are seeking a dedicated and compassionate Speech-Language Pathologist to join our team as a Geriatric Care Coordination Therapist. In this role, you will have the opportunity to practice at the top of your license while expanding your skills in care coordination.

About the Role:
This is a unique opportunity to blend your expertise in speech-language pathology with care coordination duties. You will work closely with a multidisciplinary team to provide comprehensive services to seniors in independent living, assisted living, and memory care settings.
Responsibilities:
- Provide speech-language pathology and wellness services to older adults
- Conduct assessments, treatment planning, and therapeutic interventions as part of a multidisciplinary team
- Support care coordination efforts by conducting regular wellness checkpoint visits, communicating with patients' providers and caregivers, and assisting patients with attending telehealth visits
- Maintain professional practices and ethical standards in accordance with company-wide policies and clinical initiatives
- Contribute to program development, quality improvement, and problem-solving
- Optimize each patient's functional well-being and satisfaction
Your Schedule:
While our clinics typically operate 8am to 5pm, Monday through Friday, we offer flexible scheduling to accommodate your unique lifestyle.

Requirements:
- Completion of an accredited Speech-Language Pathologist program (New Grads Welcome)
- Active ASHA certification and current state licensure in SLP
- Excellent verbal and written communication skills

Estimated Salary:
$85,000 - $110,000 per year, depending on experience and qualifications.
