Senior Care Clinician

Optum is seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem/Eugene, OR. As a clinician-led care organization, we are creating a seamless health journey for patients across the care continuum.
About the RoleIn this rewarding position, you will have the opportunity to provide high-quality care to seniors in their homes, nursing homes, and assisted living facilities. Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for patients
- Manage patients through acute medical situations or exacerbations of chronic conditions
- Collaborate with interdisciplinary care teams to ensure seamless care transitions
- Provide education and support to patients and caregivers
- Document patient information accurately and efficiently

RequirementsTo be considered for this role, you must have a graduate degree in nursing or a related field and be licensed to practice as a Nurse Practitioner or Physician Assistant in the state of Oregon.
