Senior Care Clinician

Optum is revolutionizing healthcare by providing comprehensive care to seniors in their homes and communities. We're seeking a dedicated Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem/Eugene, OR.
About the RoleAs a member of our interdisciplinary team, you'll work closely with patients, families, and caregivers to provide high-quality, patient-centered care. Your expertise will be invaluable in assessing patients' needs, developing personalized care plans, and coordinating care across settings.
Key Responsibilities- Conduct comprehensive assessments and develop patient-centered care plans
- Manage patients through acute medical situations or exacerbations of chronic conditions
- Ensure accurate and complete documentation, including ICD-10 conditions
- Communicate and collaborate with the interdisciplinary care team
- Provide patients and caregivers with counseling and education

To be considered for this role, you'll need:
- Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (for Nurse Practitioners)
- Active and unrestricted Nurse Practitioner license in the state where you reside, or ability to obtain by start date
- Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners)
- Graduate of an accredited Physician Assistant degree program (for Physician Assistants)
- Active and unrestricted Physician Assistant license in the state where you reside, or ability to obtain by start date
- Board certification by the National Commission on Certification of Physician Assistants (NCCPA)
