We are seeking a skilled Occupational Therapist to join our Home Health team. As a key member of our care team, you will work closely with patients, families, and healthcare professionals to provide personalized care and support.

Responsibilities
  • Assess and develop individualized care plans to meet the unique needs of each patient
  • Provide occupational therapy services to patients in their homes, including assessments, interventions, and education
  • Collaborate with healthcare professionals to ensure seamless care coordination and communication
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in ongoing education and training to stay current with best practices and industry developments
Requirements
  • Master's degree in Occupational Therapy from an accredited program
  • Current licensure as an Occupational Therapist in the state of practice
  • Minimum of six months of experience in occupational therapy, with a focus on home health
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
