Job Summary:The Palliative Care Clinical Nurse Coordinator will be responsible for coordinating care for patients with serious illnesses, providing education and support to patients and families, and collaborating with interdisciplinary teams to ensure comprehensive care.
Responsibilities:- Coordinate care for patients with serious illnesses, including developing and implementing individualized care plans.
- Provide education and support to patients and families, including information on disease management, treatment options, and end-of-life care.
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to ensure comprehensive care.
- Develop and maintain relationships with community resources and services to support patients and families.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- RN License in the State of Colorado.
- Compact license acceptable but must obtain Colorado RN License within 90 days of hire.
- BLS from AHA or ARC.
- Certification in Palliative Care required; OCN or CHPN required.
- Minimum 5 years of recent clinical nursing experience in oncology services; 5 years of leadership experience.
