Job Summary:The Advanced Practice Professional will be responsible for coordinating and providing clinical care to patients in the Healthy Minds Program, including assessments, treatment planning, and ongoing management. This role requires strong communication and collaboration skills, as well as the ability to work effectively with patients, families, and healthcare teams.
Responsibilities:- Manage comprehensive and individualized care for patients in the Healthy Minds Program
- Complete ACCESS clinic patient assessments and/or manage continuity patients in the Healthy Minds Program
- Develop and implement individualized plans of care for patients
- Collaborate with behavioral medicine faculty and other healthcare professionals to provide comprehensive care
- Participate in education and training programs to enhance clinical skills and knowledge
- Document patient care and outcomes in a clear and concise manner
- Master's degree in Nursing or related field
- National certification as an Advanced Practice Registered Nurse (APRN)
- Licensure as a professional Registered Nurse in West Virginia
- Experience working in behavioral medicine or a related field
- Post-Master's certification in Psychiatric Mental Health Nursing (PMHNP)
- Experience working with patients with complex behavioral health needs
