- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Conduct regular assessments and evaluations to ensure the highest level of patient care
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
- Certified Nursing Assistant (CNA) license in the state of Connecticut
- Valid CPR certification
- Licensed driver with a reliable vehicle
- Ability to work Monday - Friday, 8:00am - 5:00pm
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Competitive hourly rate of $21.00
