Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at Platinum Healthcare Corp. as a Home Health Registered Nurse. As a key member of our interdisciplinary team, you will provide comprehensive nursing care to patients in their homes, ensuring the highest quality of patient care and outcomes.

Key Responsibilities:

  • Conduct thorough assessments of patients' health conditions, needs, and abilities to develop individualized care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination and patient-centered care.
  • Administer medications, perform wound care, and provide other skilled nursing services as outlined in the care plan.
  • Educate patients and families on managing chronic illnesses, disease prevention, and health maintenance.
  • Monitor and document patient progress, reporting any significant changes in condition to the healthcare team and physicians.
  • Ensure compliance with state, federal, and agency regulations and guidelines.
  • Maintain accurate and detailed patient records, documenting all care provided in a timely manner.

Qualifications:

  • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • Licensure: Current State license as a Registered Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC, current Driver's License.
  • Experience: One year of experience as a Registered Nurse in a clinical care setting or home health preferred but not required.
  • Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR.
  • Transportation: Reliable transportation and valid auto liability insurance.
