The Advanced Practice Registered Nurse (APRN) - Primary Care position at the Veterans Affairs, Veterans Health Administration is a critical role in providing high-quality patient care. As an APRN, you will work collaboratively with the Primary Care Team to deliver evidence-based care, promote health, and prevent disease.
Responsibilities- Manage a panel of patients with acute and chronic health conditions, emphasizing health promotion and disease prevention.
- Develop and implement individualized plans to achieve optimal population health and disease management.
- Lead and collaborate with interdisciplinary teams to improve patient outcomes.
- Order and interpret lab tests, diagnostic studies, and prescribe pharmacologic and non-pharmacologic interventions.
- Initiate referrals and consults to specialty services, providing education to patients and their families.
- Assess and adjust patient plans as needed to achieve optimal outcomes.
- English Language Proficiency: Must be proficient in basic written and spoken English.
- Education: Must have a post-master's certificate or master's or doctoral degree as a Nurse Practitioner.
- License and Certification: Must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
- Basic Certification: Must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
This is a full-time position with a regular schedule of 8:00am-4:30pm. Telework and virtual work options are not available for this position.
