We are seeking a highly skilled Licensed Mental Health Professional to join our team at Fairview Health Services. As a Crisis Intervention Specialist, you will play a critical role in providing timely and compassionate care to individuals in crisis.
Key Responsibilities- Provide crisis call support and determine appropriate level of care for clients
- Conduct comprehensive behavioral assessments to identify patient needs and recommend a level of care
- Provide psychotherapy for clients and assess and complete the DEC (Diagnostic Evaluation Center) assessment form for all crisis patients
- Determine appropriate disposition, coordinate care, and provide discharge summaries for patients
- Collaborate with patients, medical doctors, and families to create treatment plans based on medical necessity and patient preference
- Provide primary communication to collaborating partners, including hospital emergency departments, schools, primary care clinics, and other treatment providers
- Adhere to all policies and procedures and maintain knowledge of available community resources and procedures for hospitalizations
- Record keeping and documentation of patient information, crisis or treatment issues, and expected outcomes
- Master's Degree in Social Work, Psychology, or closely related discipline
- Basic Life Support Certification
- Valid Minnesota driver's license and access to a car during designated hours
- Licensed in one of the following in the State of Minnesota: Licensed Psychologist, Licensed Independent Clinical Social Worker, Licensed Marriage Family Therapist, or Licensed Professional Clinical Counselor
- Good working knowledge of electronic health records and comfort level with computer-based platforms for assessments
