Nursing Process:
  • Assessment & Planning: Conduct comprehensive assessments of patients, identifying their needs and developing personalized care plans.
  • Implementation: Deliver skilled nursing care according to established protocols and physician orders, ensuring patient safety and well-being.
  • Evaluation: Monitor patient progress, document findings, and adjust care plans as needed to achieve optimal outcomes.
Responsibilities:
  • Provide direct nursing care to patients in their homes, including medication administration, wound care, and health education.
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
  • Document patient assessments, interventions, and outcomes accurately and thoroughly.
  • Maintain a safe and clean environment for patients and yourself.
  • Adhere to all applicable laws, regulations, and ethical standards.
Qualifications:
  • Graduate of an accredited registered nursing program.
  • Current Registered Nurse (RN) license in Pennsylvania.
  • Minimum one year of general nursing experience with home health, medical-surgical, or critical care experience preferred.
  • Valid driver's license and reliable transportation.
  • CPR certification.
