About the Role:

We are looking for a skilled and experienced Registered Nurse to provide high-quality patient care in a home health setting. As a Registered Nurse, you will be responsible for making intermittent home visits to our skilled nursing patients, providing care and support to help them achieve their maximum potential.

Responsibilities:
  • Provide safe and timely care to patients using acceptable nursing skills and best practices.
  • Participate in weekly team meetings to coordinate patient care and manage around COVID safety measures.
  • Develop and implement individualized treatment plans to facilitate patient rehabilitation and efficient outcomes.
  • Provide quality, compassionate nursing care while coordinating with other members of the interdisciplinary treatment team.
  • Monitor and administer medications as ordered by the patient's Primary Care Physician.
  • Conduct patient assessments and develop plans of care with other interdisciplinary team members.
  • Supervise Licensed Practical Nurses and Home Health Aide staff as needed.
  • Document all patient care, including progress notes, summaries, and discharges, per state and federal requirements.
Requirements:
  • Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the applicable state.
  • One year of nursing practice in a patient care setting, with home health, geriatrics, or other relatable settings preferred.
  • Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy.
  • Current CPR certification.
