Job Summary:

We are seeking a skilled nurse practitioner to provide primary care services to patients in their homes. The successful candidate will be responsible for conducting initial assessments, developing care plans, and managing patient conditions to ensure optimal health outcomes. You will work closely with our multidisciplinary team to coordinate care, address patient needs, and provide education on disease management and prevention.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' medical and social needs
  • Develop and implement individualized care plans to address patient goals and objectives
  • Manage chronic and acute conditions, including diabetes, hypertension, and cardiovascular disease
  • Prescribe medications and order diagnostic tests as needed
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction
  • Stay up-to-date with the latest medical research, best practices, and industry developments

Requirements:

  • Current nurse practitioner license in good standing
  • Completion of an accredited nurse practitioner program
  • Experience in primary care, home health, or gerontology
  • Strong clinical skills, with ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment and adapt to changing circumstances
