LHC Group is seeking a skilled Physical Therapist to join their team in providing high-quality home health care services.

Job Summary

The Physical Therapist will be responsible for assessing and evaluating patient care needs related to functional status, mobility, endurance, and pain management. Based on this assessment, the Physical Therapist will develop and implement a treatment plan to improve the patient's well-being and functional abilities.

Key Responsibilities
  • Evaluate patient's functional status and physical therapy needs, consulting with physicians to develop a therapy plan of care.
  • Provide services within the scope of practice, in accordance with the plan of care, and in coordination with other healthcare team members.
  • Observe and report patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.
  • Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice and evidence-based techniques.
  • Instruct and inform participating members of the healthcare team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
  • Visit patients within the entire service area of their respective agency and other nearby LHC agencies as needed.
  • Complete progress notes for each visit and submit them per LHC policy.
  • Participate in staff conferences and committees as requested by the agency.
  • May supervise home health aides per LHC policy if allowed per state regulation.
  • Provides supervision of the PTA as per LHC policy and state regulation.
  • Schedule flexibility is needed in this role to ensure we provide care to the patient as needed.
Requirements
  • Current Physical Therapy licensure in the state of CT.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
