Job Title: Physical Therapist - Skilled Nursing Facility

Supplemental Health Care is seeking a skilled Physical Therapist to join our team at a partnering skilled nursing facility in Trumbull, Connecticut.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality patient care and services at our skilled nursing facility. As a Physical Therapist, you will be responsible for assessing patients' physical abilities, developing and implementing individualized treatment plans, and providing education to patients and their families.

Responsibilities:
  • Assess patients' physical abilities and develop individualized treatment plans
  • Provide physical therapy services to patients, including exercises, stretches, and other interventions
  • Monitor patients' progress and adjust treatment plans as needed
  • Communicate with patients, families, and healthcare teams to ensure seamless care
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
Requirements:
  • Active Physical Therapist licensure/certification in Connecticut
  • American Heart Association BLS certification
  • At least 1 year of experience as a Physical Therapist
  • Prior experience in an SNF preferred
