Job Summary

We are seeking an experienced Licensed Practical Nurse to support our Vista Care Intellectual & Developmental Disability Services operations in Adams, WI. The LPN will provide education to direct care staff, conduct onsite nursing assessments, and communicate with local hospitals and pharmacies as needed.

Responsibilities
  • Report to the Registered Nurse
  • Provide education to direct care staff on medication administration, change in condition, nurse delegated tasks, and medical protocols
  • Provide nursing case management for individuals receiving our services
  • Complete onsite nursing assessments as needed
  • Communicate with local hospitals, pharmacies, and primary care physicians as needed
  • Complete various audits to ensure quality of care
  • Complete all necessary documentation
  • Ensure compliance with state and federal regulations and accreditation requirements
Requirements
  • Hold a valid Licensed Practical Nurse license in the state applying or within the compact
  • Hold a valid driver's license with a good driving record
  • Ability to write and correspond in a legible manner, operate general office equipment, and maintain technical skills with Google
  • Ability to lift 50 pounds on a regular basis, bend, twist, reach, and stand on a continual basis
