We are seeking a highly skilled and experienced Registered Nurse to join our team at John Manchin Sr. Health Care Center in Fairmont, West Virginia.

About the Role

This is a full-time position responsible for managing the Minimum Data Assessment (MDS), ensuring accuracy and completion of all facility Minimum Data Assessments prior to submission to CMS. The successful candidate will also develop resident care plans, modify care plans and services based on resident needs, and chair weekly Interdisciplinary Care Plan meetings.

Responsibilities

  • Manage the MDS process, including certification and submission to CMS
  • Develop and implement resident care plans, modifying them as needed
  • Chair weekly Interdisciplinary Care Plan meetings
  • Participate in QAPI meetings and Care Conferences
  • Orient and train new staff members

Requirements

  • Current West Virginia license to practice as a registered nurse
  • Three years of full-time or equivalent part-time experience in nursing
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
